What document does Ridgeline need to start services?
We need the completed referral form, signed Service Agreement, and active POS authorization before any payments or payroll can be processed.
Cna you pay Indepenent Facilitators (IFs)?
Yes. IFs are paid under service code 009. We require a completed referral, POS authorization, and a valid W-9 packet before issuing payment.
What is FMS burden rate?
Co -Empoyer: 18% Sole Employer: 15% This includes taxes, paid sick leave, ad workers' comp (for Co-Employer).
